_Sunglo RGC_ is looking for an LVN that will assume the responsibility for the care of patients in their homes, through the provision of comprehensive skilled nursing services.
The LVN will provide nursing care through the utilization of the home assessment, nursing diagnosis and care plans.
```Duties: ```
- Provide direct patient care in their residence
- Administer medications and treatments as prescribed by physicians or healthcare providers
- Monitor patients' vital signs and report any changes or abnormalities to the appropriate medical personnel
- Assist with patient assessments, including taking medical histories and performing physical examinations
- Perform basic laboratory tests, such as phlebotomy and specimen collection
- Triage patients and prioritize care based on their needs
- Provide education and support to patients and their families regarding their healthcare needs
- Assist with procedures, such as wound care, tube feedings, and catheterizations
- Adhere to infection control protocols to ensure patient safety
```Experience: ```
- Minimum of 2 years of experience as a Licensed Vocational Nurse (LVN)
- Experience in pediatrics, acute care, or long-term care is preferred
- Proficiency in medication administration and dosage calculations
- Familiarity with electronic health record systems (e.
g.
, KINNSER) for documentation purposes
- Ability to phlebotomize and perform basic laboratory tests
- Knowledge of infection control practices and protocols
